No Dental Insurance? We Have You Covered!

We understand that many employers are no longer offering dental insurance coverage as part of their benefit packages. Others offer dental insurance that may not be adequate to meet your oral health care needs. To address these concerns, we are happy to offer our in-house SMART Smile Membership Plan. For a low annual fee, you will receive full coverage for necessary preventive dental exams and teeth cleanings as well as discounted pricing on a wide range of other services even those not always covered by traditional dental insurance plans. In short, the SMART Smile Membership Plan allows you to reclaim power over your dental care plan. Compared with traditional dental insurance policies, there are many advantages to coverage under the SMART Smile Membership Plan. Some of the many benefits include the following:

Insurance companies are not allowed to interfere with clinical decision‑making or pressure providers to use cheaper, lower quality materials just to save the insurance company money
No monthly premium
No annual maximum

15% discount for dental services paid with cash or check
10% discount for dental services paid with credit or debt card
*separate 3% processing fee for credit card usage

For children younger than 14 years old, an annual SMART Smile Membership
Plan for a low, annual fee, children receive all of the following benefits

2 dental exams each year
2 professional teeth cleanings each year
1 set of bitewing x-rays for cavity detecting
1 Panorex x-ray every 3 years
2 fluoride treatments each year
1 emergency exam and x-ray
Complimentary CariVu images as needed

Adult
Plan

For patients aged 14 and over, an annual SMART Smile Membership Plan for a low annual fee, patients receive all of the following benefits.

2 dental exams each year
2 professional teeth cleanings each year
1 set of bitewing x-rays for cavity detecting
1 Panorex x-ray every 5 years
2 fluoride treatments each year
1 emergency exam and x-ray
Oral cancer screening
Complimentary CariVu images as needed

Periodontal Maintenance Plan

For those patients struggling with gum disease, we offer an annual SMART Smile Membership Plan for a low annual fee, patients receive all of the following benefits:

2 dental exams each year
4 periodontal cleanings each year
1 set of bitewing x-rays for cavity detecting
1 Panorex x-ray every 5 years
2 fluoride treatments each year
1 emergency exam and x-ray
Oral cancer screening
Complimentary CariVu images as needed

Terms and Conditions

The SMART Smile Membership Plan is not a dental insurance plan, and it cannot be used in any of the following situations:

In conjunction with another discount plan or dental insurance policy
In combination with services covered by workman’s compensation, medical, or general liability insurance plans
To cover any treatment that, in Dr. Moah’s sole opinion, falls outside his capabilities
For specialist referrals
For hospital charges of any kind
To purchase products

Payment Terms

Your annual fee for membership is due in full on your joining date. However, paying by credit or debit card will have a service fee added to your membership price. In order to receive the 15% treatment discount, payment for service is due in full with cash or check. When paying with a credit card, debit card or HSA/FSA, patients will only receive a 10% discount. Your plan will renew annually on the original enrollment date. No refunds will be provided after dues are paid, including requested reimbursements due to failure to schedule and keep checkup appointments. Costs of annual dues are subject to change.
